1.ਉਮੀਦਵਾਰਾਂ ਨੂੰ ਇਹ ਯਕੀਨੀ ਬਣਾਉਣਾ ਚਾਹੀਦਾ ਹੈ ਕਿ ਉਹ ਯੋਗਤਾ, ਅਨੁਭਵ, ਰਜਿਸਟ੍ਰੇਸ਼ਨ ਪੰਜਾਬ ਮੈਡੀਕਲ ਕੌਂਸਲ/ਐਨਐਮਸੀ, ਪੰਜਾਬ ਫਾਰਮੇਸੀ ਕੌਂਸਲ ਅਤੇ ਪੰਜਾਬ ਨਰਸਿੰਗ ਰਜਿਸਟ੍ਰੇਸ਼ਨ ਕੌਂਸਲ, ਮੈਟ੍ਰਿਕ ਤੱਕ ਪੰਜਾਬੀ, ਉਮਰ ਦਾ ਸਬੂਤ ਆਦਿ ਨਾਲ ਸਬੰਧਤ ਆਪਣੇ ਅਸਲ ਪ੍ਰਸੰਸਾ ਪੱਤਰ ਅਤੇ ਜਮ੍ਹਾਂ ਕਰਵਾਉਣ ਲਈ ਅਜਿਹੇ ਸਾਰੇ ਦਸਤਾਵੇਜ਼ਾਂ ਦੀ ਇੱਕ ਸਵੈ-ਪ੍ਰਮਾਣਿਤ ਫੋਟੋ ਕਾਪੀ ਲਿਆਉਣ।
2.ਉਮੀਦਵਾਰਾਂ ਨੂੰ ਇੰਟਰਵਿਊ ਦੇ ਸਮੇਂ 2 ਨਵੀਨਤਮ ਪਾਸਪੋਰਟ ਆਕਾਰ ਦੀਆਂ ਫੋਟੋਆਂ ਨਾਲ ਲਿਆਉਣੀਆਂ ਚਾਹੀਦੀਆਂ ਹਨ।
3.ਉਮੀਦਵਾਰਾਂ ਨੇ ਮੈਟ੍ਰਿਕ ਸਟੈਂਡਰਡ ਤੱਕ ਪੰਜਾਬੀ ਵਿਸ਼ਾ ਪਾਸ ਕੀਤਾ ਹੋਣਾ ਚਾਹੀਦਾ ਹੈ।
4.ਜੇਕਰ ਕੋਈ ਉਮੀਦਵਾਰ ਇੰਟਰਵਿਊ ਦੇ ਸਮੇਂ ਜਾਂ ਸਕ੍ਰੀਨਿੰਗ ਜਾਂ ਚੋਣ ਕਮੇਟੀ ਦੁਆਰਾ ਪੂਰੀ ਸੂਚੀ ਪ੍ਰਕਿਰਿਆ ਦੌਰਾਨ ਕਿਸੇ ਵੀ ਸਮੇਂ ਯੋਗਤਾ/ਚੋਣ ਦੇ ਮਾਪਦੰਡਾਂ ਨੂੰ ਪੂਰਾ ਨਹੀਂ ਕਰਦਾ ਪਾਇਆ ਜਾਂਦਾ ਹੈ, ਤਾਂ ਉਸਦੀ ਉਮੀਦਵਾਰੀ ਉਕਤ ਸੂਚੀ ਲਈ ਰੱਦ/ਰੱਦ ਕਰ ਦਿੱਤੀ ਜਾਵੇਗੀ।
